Terms & Conditions

These terms and conditions apply to the Massage Envy Cortiva Institute Massage School Scholarship Program (“Contest”) conducted and promoted by Massage Envy Franchising, LLC, 14350 N. 87th St., Suite 200, Scottsdale, AZ 85260 (“Sponsor”). By participating in this Contest, as either an entrant or a referring party (“Entrant”), you agree to the Terms and Conditions stated herein. Entrants who are selected by random number generator to receive one of fifteen scholarships (“Winners”) are subject to additional requirements in order to maintain eligibility. By participating in this Contest, you agree to the Terms and Conditions governing ongoing Winner eligibility.


  1. Entrants may not be enrolled at a Cortiva Institute at the time of entry, must be 18 years of age by the expected graduation date based on program selected and must be able to provide valid ID to confirm OR a parent or legal guardian must be willing to co-sign enrollment agreement if Entrant is selected as a Winner.

  2. If selected as a Winner, Entrants must provide proof of completion of compulsory K-12 education prior to completion of program enrollment (can be high school diploma; GED equivalent; home-study certificate or transcript from a home study program that is equivalent to high school level and is recognized by the student’s home state; proof of higher education credentials earned such as an AS, AA, BA, BS, MBA, or PhD); or college transcripts that clearly identify the high school attended and the graduation date.

  3. Winners must complete enrollment at the selected participating Cortiva Institute campus by February 8, 2019

Additional Terms

    • In order to maintain eligibility, Winners must:

      1. Remain continuously enrolled in a massage therapy program at the selected participating Cortiva Institute campus.
      2. Maintain a minimum attendance record of 85% and a passing GPA of 3.0
      3. Fulfill all requirements for massage therapy program completion within 12 months of the program start date

      By entering this Contest, Entrants agree to allow Sponsor to access school attendance and performance data relevant to determining adherence to the ongoing eligibility requirements above. If a Winner fails to meet ongoing eligibility requirements at any point between the awarding of a scholarship and the exhaustion of the scholarship award amount, the Winner will be notified, disqualified from the Contest, and any remaining award amount will be forfeited.

    • Submit an entry via web form on the Contest information page during the Entry Period (as defined below). Entrants will be required to provide:

      1. First and last name
      2. Valid e-mail address
      3. Valid phone number
      4. State and zip code of residence

      Limit one (1) entry per person, per e-mail address. Multiple or duplicate entries from the same individual will be disqualified. Use of automated entry or similar devices to participate or subvert the entry process is prohibited, and all entries through such devices are void. Incomplete, inaccurate, unintelligible or late entries that otherwise do not comply with these Terms and Conditions will be disqualified. Entries are the property of the Sponsor and will not be acknowledged or returned. Entrants should be able to provide on request proof of eligibility. All entries must be received during the Entry Period (as defined below) to be eligible.

      IMPORTANT NOTE: No Entrant will be eligible to receive a scholarship unless Sponsor determines, in its sole and absolute discretion that all requirements for eligibility and as otherwise set forth in these Terms and Conditions have been met.

    • All entries become the property of Sponsor and may not be acknowledged or returned. Each Entrant hereby acknowledges and agrees that the relationship between the Entrant, on the one hand, and Sponsor, nor any of its past, present, or future affiliates or subsidiaries and their respective officers, directors, incorporators, members, partners, owners, agents, management, controlling parties, entities under common control, vendors, service providers, attorneys, employees, or representatives ('Sponsor Entities'), on the other hand, is not a confidential, fiduciary, or other special relationship, and that the Entrant's decision to enter Contest does not place Sponsor or the Sponsor Entities in a position that is any different from the position held by members of the general public with regard to elements of the individual's entry.

      Sponsor and the Sponsor Entities are not responsible for any problems or technical, hardware, or software malfunctions of any telephone network or telephone lines, failed, incorrect, inaccurate, incomplete, garbled or delayed electronic communications whether caused by the sender or by any of the equipment or programming associated with or utilized in this Contest, computer online systems, servers or providers, computer equipment, software, failure of any e-mail or entry to be received by Sponsor due to technical problems, human error or traffic congestion, unavailable network connections on the Internet or at any website, or any combination thereof, including, without limitation, any injury or damage to Entrant's or any other person's computer relating to or resulting from participating in this Contest. By submitting an entry, Entrant agrees to abide by the Terms and Conditions of the Contest.

    • At the close of the three-week entry period between January 7, 2019 through January 28, 2019 (the “Entry Period”), a total of fifteen scholarship Winners will be randomly selected from among all applications received through the online scholarship application form by midnight Pacific Time on Monday, January 28, 2019 using a random number generator.

    • Fifteen (15) $1,000 scholarships will be awarded based on random selection process described above. These scholarships may only be used for program costs associated with massage therapy programs at Cortiva Institute campuses of Arizona, Texas, and Florida. Scholarships are not assignable or transferable. Scholarships will be paid directly to the participating Cortiva Institute campus at which a Winner ultimately enrolls until the scholarship amount is exhausted or a Winner fails to meet ongoing eligibility criteria. Winner is responsible for paying all applicable program costs not covered by the scholarship and any difference between the scholarship award amount and actual payment required to be paid to the applicable Cortiva Institute campus will not be retained by the Sponsor.

    • Eligibility, age, claims and proof of enrollment in a degree, diploma, or certificate seeking program are subject to verification. Winners will be notified by telephone and/or e-mail after the close of the Entry Period and may be required to sign an affidavit of eligibility, FERPA waiver, liability release, and/or publicity release, which Winners must sign as a condition to the award of any scholarship. In the event a Winner fails to provide a written acknowledgement of acceptance of the award of a scholarship within five (5) days of being notified of such award, or fails to return signed affidavits/releases if required by Sponsor within five (5) days of notification, the Winner will be disqualified, and alternate Winners may be selected through the same process described above at the Sponsor’s discretion. Scholarship payments will be made directly to participating Cortiva Institute campuses at which Winners are enrolled on a payment schedule mutually agreed upon by the Sponsor and participating Cortiva Institute campus once eligibility and compliance with these Terms and Conditions are verified. Sponsor is not obligated to leave voice mail, answering machine or other messages. Sponsor is not responsible for fraudulent calls or e-mails made to Entrants not by the Sponsor. Scholarship will be awarded only if they are properly acknowledged and claimed. In the event of a dispute as to the identity or eligibility of a Winner based on an e-mail address, the winning entry will be declared as the 'Authorized Account Holder' of the e-mail address at time of entry. An 'Authorized Account Holder' is defined as the natural person 18 years of age or older who is assigned to an e-mail address by an Internet access provider, on-line service provider, or other organization (e.g., business, education institution, etc.) that is responsible for assigning e-mail addresses for the domain associated with the submitted e-mail address. Sponsor may ask any Entrant or potential Winner to provide Sponsor with proof that such party is the authorized account holder of the email account associated with the entry.

    • If for any reason the Contest is not capable of running as planned due to an infection by a computer virus, bugs, tampering, unauthorized intervention, fraud, technical failures, or any other causes beyond the control of Sponsor or the Sponsor Entities that corrupt or affect the administration, security, fairness, integrity, or proper conduct of Contest, the Sponsor reserves the right at its sole discretion, to disqualify any individual who tampers with the entry process, and to cancel, terminate, modify or suspend Contest.

    • By participating in the Contest, Entrants: (1) agree to be bound by these Terms and Conditions; (2) agree to release Sponsor and the Sponsor Entities from any and all liability for any loss, harm, damages, costs or expenses, including without limitation property damages, personal injury and/or death, due in whole or in part, directly or indirectly, to participation in Contest or any Contest-related activity, or the acceptance, possession, use or misuse of any scholarship and claims based on publicity rights, defamation or invasion of privacy and merchandise delivery; and (3) consent to the use of their names, voices, pictures, and likenesses for advertising purposes in any medium throughout the world, whether now known or hereafter devised, in perpetuity without additional compensation or right of review unless prohibited by law, (4) consent to the use of their entry responses for promotional purposes in any medium throughout the world, whether now known or hereafter devised, in perpetuity without additional compensation or right of review unless prohibited by law,(5) agree that Sponsor owns all data and responses collected through this Contest and (6) Sponsor may discontinue Contest at any time. Sponsor may terminate any Contest or decide to modify it in any manner that it deems fair and equitable to participants if it becomes aware of any occurrence that would materially affect the Contest’s security and fairness. Sponsor may prohibit individuals from participating in the Contest and disqualify entries if they (1) attempt to apply for the scholarship through any means other than as described in these Terms and Conditions, (2) attempt to disrupt Contest or circumvent the Terms and Conditions in any way or (3) violate these Terms and Conditions. All of Sponsor's decisions with respect to the Contest are final.

    • By participating in the Contest, Entrants agree to release and hold harmless Sponsor and the Sponsor Entities from and against any claim or cause of action arising out of participation in the Contest or receipt or use of any scholarship hereunder, including, but not limited to: (a) unauthorized human intervention in Contest; (b) technical errors related to computers, servers, providers, or telephone or network lines; (c) printing errors; (d) errors in the administration of Contest or the processing of entries; or (e) injury or death to a person(s) or damage to property which may result, directly or indirectly, in whole or in part, from Entrant's participation in Contest or Entrant's receipt, acceptance, use and/or enjoyment of any scholarship. In no event shall Sponsor or the Sponsor Entities be liable for attorney's fees. Each Entrant waives the right to claim any damages whatsoever, including, but not limited to, punitive, consequential, direct, or indirect damages. Sponsor and the Sponsor Entities reserve the right to correct typographic, clerical or printing errors in any Contest materials. Sponsor and the Sponsor Entities are not responsible for any typographical or other error in the printing of Contest, the administration of Contest or in the announcement of the Winners, including such error as may give an erroneous indication that an award has been won. In the event the Sponsor or the Sponsor Entities are prevented from continuing with the Contest, or the integrity and/or feasibility of the Contest is undermined by any event including but not limited to fire, flood, epidemic, earthquake, explosion, labor dispute, or strike, act of God or public enemy, satellite or equipment failure, riot or civil disturbance, war (declared or undeclared), terrorist threat or activity, or any federal, state, or local government law, order of regulation, order of any court or jurisdiction, or other cause not reasonably within the Sponsor's or the Sponsor Entities' control (each a 'Force Majeure' event) the Sponsor and the Sponsor Entities shall have the right, in their sole discretion, to abbreviate, modify, suspend, cancel or terminate the Contest without notice or further obligation. If the Sponsor or the Sponsor Entities, in their discretion, elect to abbreviate, cancel or terminate the Contest as a result of a Force Majeure event, the Sponsor and the Sponsor Entities reserve the right, but shall have no obligation, to award the scholarships from among all valid and eligible entries received up to the time of such Force Majeure event.

    • The information provided in connection with the Contest is being disclosed solely to Sponsor and the Sponsor Entities, and may be used for Sponsor's internal, promotional, and advertising purposes and to send Entrants information from the Sponsor and/or Sponsor Entities. The submission of an entry constitutes permission to post each Winner's name on Sponsor's website and to use such Winners' names, states and/or likenesses for purposes of advertising without further compensation or right of review unless prohibited by law. Clear avenues will be made available for Entrants to unsubscribe from all further communication if desired.
